Form 1095-C

This form reports the employee-level medical coverage information for fully-insured ALEs. It is sent to employees for their records. The employer also files it with the IRS.

This form acts as the cover letter for the employer. It is submitted by the employer to the IRS together with the Forms 1095-C.

This form reports the employee-level medical coverage information for self-funded companies. It is sent to employees for their records. The employer files it with the IRS.

For fully-insured groups, the coverage provider reports and sends the 1095-B to employees for their records.

Form 1095-A

This form is issued by the Health Insurance Marketplace, also known as the Health Exchange. It reports on enrollment in qualified plans. Individuals receiving health insurance through the Exchange will also need this form for their taxes.
